Past Ocular History Congenital cataracts OU Chronic angle closure glaucoma OS Anophthalmos OD Pseudophakia OS Ocular surgery OD Needling of congenital cataract 1938 Enucleation 1954 Secondary orbital implant 4/2011 Ocular surgery OS Needling of congenital cataract 1938 DSAEK/Phaco/Intraocular lens x 2/Anterior vitrectomy 2/2007 Ahmed valve Seton model FP-7 with scleral patch graft 3/2007 complicated by hypotony and choroidal effusion Past Medical History: Osteoarthritis, hypertension, hyperlipidemia, atrial fibrillation, depression Medications: Alprazolam, artificial tears in the left eye as needed, ascorbic acid, celecoxib, cholecalciferol, hydrocodone/aspirin, metoprolol, prednisolone 1 drop in the left eye daily, rosuvastatin, sertraline, tramadol, warfarin Allergies: No known drug allergies. Family History: Patient’s son has glaucoma. Social History: Patient has a history of prior tobacco use, occasional alcohol use. Review of Systems: Pertinent findings are in the HPI.
